Youth mentoring and behaviour support for young people with disability, trauma, or both.
Specialist one-to-one support for young people aged 12 to 20 across Western and South West Sydney. Trauma-informed. Neuroaffirming. NDIS-funded and self-referred families welcome.
Four ways we support young people.
Every service is trauma-informed, delivered by qualified practitioners, and designed to build capacity, not compliance. Available NDIS-funded and, in some cases, self-funded.
Youth Mentoring
Trauma-informed one-to-one mentoring in the community, home, and school. Rapport before skill. We work with autistic young people, ADHD, complex trauma, and OOHC placements: the profiles most providers hand back.
Specialist · Youth-onlyPositive Behaviour Support
PBS delivered personally by Yos, an NDIS behaviour support practitioner. Neuroaffirming, restrictive-practice-reducing, and specialised for young people 12 to 20.
Real skill, not busyworkNDIS Capacity Building
Life skills, community participation, and independence work that maps to specific NDIS goals. We build capacity you can point to at the plan review, not hours you had to justify.
DCJ & foster care partnersOOHC Youth Support
Specialist mentoring for young people in kinship, foster, and residential care. Continuity of practitioner, weekly case notes, and the reporting cadence DCJ caseworkers actually need.
Rapport before skill. Capacity before compliance.
Every session is built on the Elevate Youth Lab model. We invest in relationship first, because no learning happens without it. Then we build capacity that lasts.
Trauma-informed as default
Behaviour is communication. We do not treat behaviour without asking what it is trying to tell us. Every practitioner is trained in complex trauma practice.
Neuroaffirming, always
We work with how a young person's brain is wired, not against it. Autistic, ADHD, disability, and trauma responses met on their own terms.
Capacity before compliance
We do not aim for a quiet young person. We aim for a capable one. Compliance without capacity fails the young person and the system.
Across Western and South West Sydney.
Our team travels to homes, schools, and community spaces so young people can be supported where they already are.
Two pathways in. Same standard of care.
You do not need to have an NDIS plan to work with us. If a young person needs support and can benefit from what we do, we will find a way.
NDIS-funded pathway
If the young person has an NDIS plan, our mentoring and life-skills work is typically funded under registration groups 0125 Participate in Community, Social and Civic Activities, 0117 Development of Daily Living and Life Skills, and 0116 Innovative Community Participation. We work with self-managed, plan-managed, and NDIA-managed plans.
- Registered NDIS provider (Certificate 4-KQ545PP)
- Direct billing for plan-managed and NDIA-managed
- Detailed reports for plan reviews
Self-referred and privately funded
Not everyone with a disability has an NDIS plan. Not everyone with trauma qualifies for one. If the young person needs mentoring or behaviour support and there is no plan, we offer self-funded packages, referral pathways to advocacy, and support to explore eligibility when it makes sense.
- No NDIS plan required
- Transparent hourly rates
- Advocacy referrals if you want to explore NDIS eligibility
Wherever you sit in the system, we can help.

Is SCA a registered NDIS provider?
Yes. Supporting Communities Australia is a registered NDIS provider based in Western Sydney. We work with self-managed, plan-managed, and NDIA-managed participants.
Who do you work with?
Young people aged 12 to 20 with disability, trauma, or both. Our practice is trauma-informed and neuroaffirming by default. That includes autistic young people, young people with ADHD, young people in out-of-home care, and young people carrying complex trauma from any origin, whether or not they have an NDIS plan.
Do I need an NDIS plan to work with you?
No. Most of our work is NDIS-funded, but we accept self-referred and privately funded families too. If a young person needs support and can benefit from what we do, we will find a pathway. We also help families explore whether NDIS eligibility might be worth pursuing.
What suburbs do you cover?
We deliver services across Western and South West Sydney including Campbelltown, Liverpool, Fairfield, Parramatta, Blacktown, Bankstown, and surrounding suburbs. Our team travels to schools, homes, and community settings.
Do you work with young people in out-of-home care?
Yes. OOHC youth support is a specialist area. We partner with DCJ caseworkers, foster carers, and residential care teams to provide continuity, mentoring, and behaviour support.
How can NDIS funding pay for mentoring?
Youth mentoring is fundable under Capacity Building registration groups including 0125 Participate in Community, Social and Civic Activities, 0117 Development of Daily Living and Life Skills, and 0116 Innovative Community Participation. Ask your support coordinator to confirm which group your plan uses.
